Getting Started
Welcome to my booking page. Here you will find everything you need to get started. If you are a new client, please begin by completing the intake form so I can learn more about your horse or dog. This helps me provide safe, effective and tailored treatment. You will also find a contact form further down the page which you can use if you would like to ask questions, discuss your animal’s needs or go ahead and book your first appointment, whatever feels right for you.
New Client Form
Before your first session, I ask all new clients to complete a short intake form. This gives me important information about your horse or dog, their health history, workload, and any current concerns. It also allows me to contact your veterinary surgeon for consent, which is a legal requirement before physiotherapy can begin. By filling out this form in advance, we can make the most of your appointment time and ensure your animal receives safe, effective, and tailored treatment.
